00:04:26.660 Yeah. And I think that's great. You know, we as women, a lot of times we focus on everyone else.
00:04:31.120 So I don't think we advocate for ourselves enough. And then on top of that, these are
00:04:36.080 sensitive subjects we're talking about. People are judging whether they should be or not.
00:04:40.520 So it's hard to have these conversations to feel comfortable and safe having them.
00:04:44.200 Yeah, it's totally true. So I started on a GLP, oh my gosh, I guess about a year and a half ago. And when I did that, it was still not totally OK to talk about all the time. Like people were like, well, you shouldn't be going on this. You're just you're getting rid of vanity weight. And how dare you? But I talked about it really openly. And women were so happy because they're like, I'm sick of feeling ashamed about this.

00:05:53.540 between a glp1 a glp2 and then oh the upcoming glp3 i know the new hot sexy uh drug right um
00:06:01.700 Though the GLP-1s hit one receptor and the GLP-2s hit that same receptor plus another receptor, side effect profile can be a little different from medication to medication.

00:06:18.960 And that is reportedly there's less side effects, higher percentages of weight loss, and as well as fat burning from that glucagon receptor.

00:07:03.220 was gross i'm like i've i hadn't experienced this since i was pregnant uh and i had those for about
00:07:09.340 eight weeks and then they started to taper off and to go away i haven't talked about this since
00:07:15.120 i've been i'm on a moderate dose now i'm kind of on a maintenance dose uh and i think i don't know
00:07:23.240 I don't know if I'm going to be on it forever, but I lost 30 pounds, 30 pounds that I had gained over having three babies in eight years in my late 30s, early 40s.
00:07:36.220 And I say this all the time. I think I went straight from making babies and breastfeeding them into perimenopause because I had my last baby at 42, right?
00:07:45.800 I stopped breastfeeding at 43. And so then all of a sudden I go straight into perimenopause.
00:07:51.120 from that. My hormones have been wacky. There's no way that I was going to lose that weight on
00:07:55.860 my own. And I was so uncomfortable. My joints hurt. My sleep was bad. I mean, my sleep is now
00:08:01.480 still bad because of the perimenopause, but my body didn't feel okay. This didn't have to do,
00:08:07.320 you know, with a lot of people saying vanity. And this had to do with, I didn't feel right in my
00:08:11.980 body. Do you hear that from a lot of patients? All the time. And a lot of it has to do with
00:08:17.240 inflammation. So that's one of the most amazing things that these medications do is the reduction
00:08:22.720 in inflammation. So let's say you weren't on one. We could draw your inflammatory markers today.
00:08:27.600 We could start you on one. And that's the first thing we would see before you lost any weight, 0.90
00:08:33.160 your inflammatory markers would come down and you would feel better because we've reduced that
00:08:37.380 inflammation. I can tell you personally that when it's been too long in between my injections,
00:08:42.460 my hip hurts. And I, you know, I even have patients who have rheumatoid arthritis and
00:08:48.040 they're controlled on their meds, but they gain some weight. They're on these two and they can
00:08:52.000 tell when it's time for another injection because they hurt again. But yeah, I think that's exactly
00:08:57.900 what I'm hearing from people. Yep. That is so fascinating. And I don't know if we talk about
00:09:02.580 this enough, but like my body hurts less after being on the GLPs. What do these drugs do? Why
00:09:09.840 does it decrease the inflammation in our bodies by regulating our response to glucose and how our
00:09:18.760 body is managing all of that it also you know on some level helps regulate other hormones so that
00:09:25.100 helps too and when we get into talking about hormone replacement therapy you know therapy
00:09:29.520 both you know estradiol and testosterone can affect our inflammation and joint pain and all
00:09:35.960 in addition to a bunch of other things yeah i know and my mom who is 70 years old who's also
00:09:41.740 been on it for about nine months now she's lost 30 pounds and so i mean she's so happy and she just
00:09:49.080 she walks better now than she ever did before her body can move again she works out in the gym
00:09:55.780 every day and i'm just i'm really happy with how it's worked out for me and for most of the people
00:10:03.620 that I know. So are you on these shots too? Yes, I did start a low dose for, mostly for
00:10:10.460 anti-inflammatory benefits. We do, and we do that too. I'm sure you've heard, you know, the buzzword,
00:10:16.300 the micro dosing. We do have people on very low doses. I have not increased my dose at all because
00:10:22.020 I didn't need to lose a bunch of weight, but I feel different. I actually had someone say to me
00:10:26.900 today, what are you doing? And we were able to have that conversation, the patient I saw right
00:10:31.620 before jumping on here with you. But yeah, we have, you know, I think that's a big part of all
00:10:36.520 of this too, is how it's being dosed, the source that things are coming from, how it's being dosed,
00:10:41.980 making sure you know where, who your provider is and where these medications are coming from
00:10:48.020 is super important. Right. Right. Absolutely. And I feel like there are a lot of places that
00:10:56.780 are claiming that they're getting you these meds and you don't necessarily know where they're coming

00:11:17.100 scary thing is that you're not overthinking this. I know there are places we are very
00:11:23.260 comfortable with who our pharmacy is. They send us purity data every time they make a new batch
00:11:28.960 of the medications. But there are places, especially the online pharmacies, where they're
00:11:34.040 getting these toutered versions from China. They're mixing them up themselves, slapping their label on
00:11:39.680 it, and you have no idea what you're getting. So you're absolutely correct. You should be going to
00:11:44.280 a practice where there is close medical supervision and make sure that the practice itself is
00:11:50.060 reputable. There's a lot of people out there claiming, like you're saying, that they're

00:13:13.760 you want to debunk? You know, I think people worry about, people are always concerned about
00:13:19.780 side effects, right? We do a lot of educating. That's why I kind of mentioned that about going
00:13:25.080 to the right medical provider. I'm going to catch a lot of flack if I say you should only go to a
00:13:29.420 doctor. So that's why I kind of like skirted a real mess. Yeah, no, of course. But we do a lot
00:13:35.700 of educating with our patients. So they know they're supposed to be eating protein. They know
00:13:39.680 how many calories they're supposed to be eating. So maybe diet is a good thing to talk about,
00:13:46.020 diet side effects um am i gonna have to be on this forever yeah is a big question too i think
00:13:54.380 that's the question i still have i still have that question because when i went off of it i
00:13:58.440 started gaining weight again uh and i didn't feel good that is the other thing when i went off the
00:14:03.440 glp i think it's because of what you were talking about because of the inflammation i started to
00:14:07.860 feel crappy right and so now i am on a low dose and i feel really good again my weight is very
00:14:14.140 steady and it's I don't want to lose any more weight like I'm actually worried that if I lose
00:14:18.100 more weight I'm going to start to look like the Crypt Keeper because I'm a 45 year old woman I
00:14:21.040 have no collagen left so uh yeah I just want to maintain I feel comfortable in my body right now
00:14:27.340 and this is where I where I think I want to be uh but yeah I mean is this a thing people have to be
00:14:32.920 on forever because it's also expensive I pay out of pocket for it yeah and most people do um I would
00:14:37.520 say most of our patients probably wouldn't qualify with insurance or anything anyway so that's why
00:14:44.140 you know they come to see us because they didn't anywhere else but you're like strict about what
00:14:50.220 we do i mean we've got protocols and we do things you know but there's a lot of places that's kind
00:14:55.580 of what it's getting at you don't like you don't know what you're getting right here's your month
00:14:58.440 of dosing and you're like and they feel like crap because no one's keeping an eye on you know
00:15:03.340 right exactly like i remember i was so thirsty because my body wasn't remembering to drink

00:15:16.240 of the food noise was really nice uh you know i was i was definitely dehydrated i had to have some
00:15:21.460 electrolytes what are your recommendations for protein i find that it feels like my full-time
00:15:28.020 job to eat enough protein in a day. I don't know how anyone possibly eats their, you know, the
00:15:34.400 rights, the amount that social media tells us to eat, which is what, like one gram per pound of
00:15:41.000 weight. Every time we see people, that comes up. Hydration comes up. I mean, we talk about it all
00:15:49.220 the time. Yeah. How much protein should I be eating? A gram per pound, if you can. A gram per
00:15:56.100 How do you get that much protein? How do you do that?
00:15:59.040 It's hard without a shake or a protein bar or, you know, I mean, I don't know about you, but I can't eat that much chicken in a day to get any protein.
00:16:07.360 I can't eat that much chicken. I can't eat that. I just had two hard-boiled eggs. I can't eat any more eggs for today.
00:16:12.200 I've eaten all of the cottage cheese that I can eat. So I guess we have to do a shake, right?

00:16:21.220 And most of them are pretty good. You know, some people worry about the aftertaste. I tell people if you drink the shake I drink, you probably should be okay because my teenage son will drink it. So that's it right there that it doesn't taste enough. But yeah, a shake, a protein bar, some sort of, you just got to be careful. You know, you really have to look at those too to make sure there's not a lot of hidden sugar in them.

00:17:10.820 i'm close to it and then i just then i only have to have one tub of cottage cheese for the day right
00:17:15.740 Exactly. Yeah, no, it's not it's not easy. I mean, also, I was just talking to a girlfriend on the phone about trying to get in 10,000 steps a day. I live in a city. We don't drive anywhere in 10,000 steps is still impossible. So it does feel like there's just so many things that we should be doing. And as long as we're aiming for these things and we're trying, is that good enough?
00:17:37.860 I think for women, especially over 40, in general, resistance training is more important than your cardio. And that is huge with these medications too, because regardless if we do everything right, you're still going to lose a little bit of muscle mass. I mean, that's what we're trying to avoid, right, with the protein consumption. So doing some sort of resistance activity is really going to benefit you in the long run.
00:18:02.720 And that's the other thing, too. You know, people are like, I'm not losing fast enough. Well, if you're losing too fast, then you're losing muscle mass. And it's not just the weight that you want to be losing. So we have to keep a close eye on that, too, which is why we talk about that. We discuss it extensively at every visit with our patients.
00:18:20.540 Right. Right. So with resistance training, does that mean 30 minutes of weights a day? Or can you
00:18:26.000 be doing 10 minutes of weights a day and some resistance bands and just working this into your
00:18:30.860 life in a way that actually works for all of my friends are really busy women. Most of them have
00:18:36.680 kids. They're like, it's just so much to get everything done. Whatever you can get in. I don't
00:18:41.020 want to I think it's worse to tell people you have to do this much and then they're overwhelmed. So
00:18:45.780 they do nothing um right so whatever which is how i feel yeah work it in however it works for your
00:18:51.700 schedule but you got to do it we we have to do it and i try to practice we all everyone in my
00:18:56.440 office is on one um we all try to practice what we preach i mean i've sent patients home with my
00:19:01.700 protein shakes before so you have oh my gosh wait what what protein shakes do you drink um and i'm
00:19:06.920 not here to promote for any protein shake but uh we i drink the premier protein shakes you can get

00:19:55.080 vest. Um, but that certainly would help you get, you know, some exercise in. Um, but, uh, you know,
00:20:02.180 I think, like I said, I think resistance training is probably the most important thing any of us
00:20:05.740 can do for ourselves, especially over 40. Right, right. Yeah, yeah, absolutely. And
00:20:11.760 when you're checking in with patients to make sure they're not losing muscle,
00:20:15.720 how do you do that? How do you make sure that they are maintaining a good amount of muscle?
00:20:20.640 The first thing we start with, so there are different body scans that you can get done
00:20:24.360 that we'll show you. You know, probably best, you don't make anything a pattern, right? So
00:20:30.240 where did you start? Where are we now? But there are certain types of body scans that'll do that.

00:23:58.400 me a little bit about what you're seeing your patients go through it depends on their age but
00:24:03.280 i think there's a huge struggle because for so long and this is what we learn in school too right
00:24:09.800 in medical school, we were taught that hormones were bad because of that 2002. Well, it was when
00:24:15.700 it was stopped. The study was stopped in 2002, the Women's Health Initiative study. But those
00:24:19.780 were synthetic hormones. And so I think everyone was scared for such a long time to even prescribe

00:27:23.000 hormones for women, that is the one that is most ignored, I think, for a couple of reasons,
00:27:27.600 because people think of it as a male hormone and people don't know what to do with it. So they
00:27:33.600 don't check it. Right. Like if you don't know what to do with something, you're not going to
00:27:37.020 go looking for it. But testosterone is the most biologically active hormone even in a woman's
00:27:42.720 body. And it affects a lot more things than just sex drive. I mean, we're talking a lot of the
00:27:47.280 general symptoms you feel, joint aches, generalized fatigue, loss of muscle, weight gain, all of that
00:27:54.380 and more, not limiting the conversation that your rainfall, mental clarity, all of that stuff.
00:28:01.700 So you have to really look at the whole package when you look at hormones for men and women.

00:31:07.940 what kinds of things are you prescribing for women in my cohort? We are typically just at this point
00:31:14.320 doing pellet insertions for a couple of reasons. I'm not opposed to doing hormone replacement
00:31:22.020 therapy other ways and i have done that for people who for one reason or another did not want the
00:31:26.520 pellets um mostly just personal preference but with the pellet insertions you don't have to
00:31:32.020 remember to take something every day oh i don't want to take a pill every day or put a patch on
00:31:36.380 every so often or put a cream on that we're not sure how well you're going to absorb it and then
00:31:40.960 you can't touch someone because the cream's on your arm and again you get that higher and more
00:31:45.440 predictable, steady state of hormone in the body. So you get those systemic benefits like we were
00:31:52.500 talking about with your brain, mental clarity, the cardiovascular protection, and then bone 1.00
00:31:59.000 density, which is so important for women. So important for women. I mean, that is why I'm 0.99
00:32:04.680 lifting all the weights and huffing all of the protein all the time. Well, and too, if you look
00:32:09.840 at in terms of cardiovascular health um our heart attack rate is lower as women before menopause

00:33:44.860 are people nervous about um i think you know that study all the things that came out of that study
00:33:50.720 you know they did see an increase in heart attack stroke and death and that was because of those
00:33:55.500 synthetic hormones um i i think they're you know there's always concerns about weight gain and um
00:34:03.520 and i think but i think more importantly you know they people are just afraid to talk about it
00:34:10.480 yeah yeah no i mean i think that there's so much that we're afraid to talk about because
00:34:18.040 as women we often feel shame about like you said putting ourselves first or caring for ourselves
00:34:23.840 when we're so busy caring for other people and this is our time we cannot take care of our 1.00

00:35:17.360 going to be in the bloodstream? I still think there are questions out there about that. We know
00:35:21.440 how these injectable ones are working that they are working um and how well they're working
00:35:25.660 right right exactly yeah it's it's really funny because like i said i'm watching the pit and i'm
